Strategic Service Delivery Plan

The Strategic Service Delivery Plan (SSDP) brings together and articulates a vision shared by partner agencies for an improvment in health and a reduction in health inequalities in Southern Derbyshire through the modernisation and integration of services in primary care settings delivered through a LIFT arrangement.

The Department of Health produced the NHS Improvement Plan in March 2005 which laid down the emphasis for NHS organisations over the next 3 years:-

• Greater range of choices about services and information about services
• Stronger standards and safeguards
• Better understanding of patients and their needs ie personalised services
• Integrated networks across commissioners and providers
• Develop new primary and community services nearer to patients
• Improving patient pathways particularly for long-term conditions
• Support clinical leadership and encourage leadership at all levels
• Improved IT and information, payment by results and more locally based commissioning
